Trichloroacetic Acid (TCA) Peel Protocol

15–30% In-Office Chemical Peel Procedure
(Professional Use Only — For Supervised Application)


Overview

TCA (Trichloroacetic Acid) peels are used to treat:

  • Fine lines and wrinkles

  • Superficial scars

  • Minimal skin laxity

  • Hyperpigmentation and sun damage

TCA is safe for most skin types, though extra caution must be taken with darker skin tones (Fitzpatrick IV–VI) to avoid post-inflammatory hyperpigmentation. This peel can be applied not only to the face but also to the neck, chest, back, arms, and hands—any area where skin rejuvenation is desired.

TCA is the active ingredient in many medical-grade peels such as the Obagi® Blue Peel, which uses a 30% solution applied in multiple layers under physician supervision.


Contraindications – Do NOT perform TCA peel if the client has any of the following:

  • Excessive sun exposure or plans for upcoming exposure

  • Active Herpes Simplex (prescribe antiviral like Zovirax®)

  • Warts or active skin infections

  • History of keloid scarring

  • Recent facial surgery

  • Use of Accutane® (within the past 12 months)

  • Pregnancy or lactation

  • Autoimmune disorders, recent chemotherapy or radiation

  • Sunburn, windburn, broken skin, recent waxing or depilatory use (e.g., Nair®, Vaniqa®)


Pre-Treatment Guidelines

  • Begin a pre-treatment skin care regimen 2 weeks prior to the peel.
    Include:

    • AHAs (e.g., Glycolic or Lactic Acid)

    • Brilliance/Kojic Acid serums

    • Brightening agents to even out skin tone

  • Recommend Hyaluronic Cream & Serum for hydration

  • For post-peel care, consider topical Bacitracin if needed


Post-Treatment Expectations

After 1 Layer:

  • Client may feel a sunburn-like sensation and skin tightness

  • Use a gentle cleanser, pat skin dry, and apply a moisturizing ointment

  • Do NOT scrub or pick at the skin

After 2–3 Layers (Medical Use Only):

  • Skin may turn brown and begin shedding within 7–10 days

  • Absolutely do NOT pull or peel flaking skin — this may lead to scarring

  • Full recovery can take up to 2 weeks


Step-by-Step TCA Peel Instructions

  1. Review Warnings & Contraindications
    Ensure client is a suitable candidate and understands risks.

  2. Cleanse the Skin
    Use Tea Tree Cleanser to remove impurities. Rinse thoroughly and pat dry.

  3. Exfoliate (optional)
    Apply Exfoliating Scrub with an appropriate exfoliant. Rinse off completely.

  4. Degrease
    Apply Pre-Peel Solution using gauze to remove oils and prepare the skin.

  5. Apply TCA Peel

    • Moisten ½ of a 2” cotton pad with 1–2 ml (20–40 drops) of TCA.

    • Hold the dry half and gently apply using horizontal strokes.

    • Do not rub or press into the skin.

  6. Wait for Coagulation

    • Wait 3–5 minutes for frosting to occur.

    • For deeper peels (under MD supervision only), apply a second or third layer, waiting 3–5 minutes between layers.

    • Frosting (skin turning white) is expected at 3 layers.

    Never apply more than one additional layer per treatment.

  7. Do NOT Neutralize

    • TCA self-neutralizes within 4–8 minutes.

    • If needed, rinse with plain water to remove excess solution.

  8. Soothe the Skin

    • If the client experiences discomfort, apply a Cryogenic Peel-Off Mask after 3–5 minutes.

    • Follow with Hydra Cream & Hydra Serum for deep hydration and barrier support.

  9. Sun Protection

    • Apply Anti-Aging Cream SPF 30 immediately post-treatment.

    • Instruct the client to use SPF daily for several months post-peel.


Post-Treatment Aftercare

  • Use a gentle cleanser and moisturizing ointment daily.

  • Avoid scrubbing, exfoliating, or direct sun exposure.

  • Do not pick or peel any flaking skin.

  • Continue using Hyaluronic Serum, Hydra Cream, and SPF 30 daily.
